I’m a bit confused by the neutral/earth swap comment, and most black magic and similar switch mode supplies dont actually have a connection on the ground? Plastic pins prevent these faults? Few of the small products are big enough for internal psu’s and IECs. Have you got a temperature gun? Check the troublesome one against the others in a test and see if its hotter? Sticking  a class II power supply on a tester produces little useful info. No test to the earth/ground.
