Definitely the DSLR's shutter, you can see the same thing happen in videos of high speed things on many devices, including the iPhone 3G. Dunno about the 4/5. If the suspension were that bad you would hear it, and it would look like rocking not like a wave.

Each frame appears to be progressively scanned which means the cone has moved between starting at the top of the frame and ending at the bottom, that's what gives it the tilt.
Try an iphone on a prop airplane someday, I'm still trying to figure out the mechanics of why it looks like it does!

I took a photo of a helicopter in flight with my iPhone and was surprised to learn that the rotors were made of silly putty.
